Before your visit, please check with your insurance company to verify your level of coverage (co-pays, co-insurance, deductibles). If you are paying without the help of insurance, payment is due in full at the time of service. We will submit any insurance claims for you.
Although we will call to confirm your appointment 24 hours before its scheduled time, it is your responsibility to cancel within 48 hours.
If you do not cancel, and do not show for the appointment, you will be charged a cancellation fee of $100.
